Medical (military medical) Commission

The Medical (military medical) Commission is a regular medical (military medical) Commission of the State Institution "Territorial Medical Association of the Ministry of Internal Affairs of Ukraine in Vinnytsia region", established in the system of the Ministry of the Interior to conduct a military medical examination and medical examination of candidates for service, persons of the ordinary and superior composition of the national police, military personnel of the National Guard of the Ministry of Internal Affairs of Ukraine, civilian people who want to enter educational institutions of the Ministry of the Interior, the Ministry of Defense of Ukraine, cadets and listeners of educational institutions of the Ministry of Internal Affairs of Ukraine, former individuals of the ordinary and superior composition of the internal bodies of internal Affairs, former police and former servicemen.

To pass a medical examination, candidates for service need to provide:

1. Directions to M (MM) C.
2. Passport of a citizen of Ukraine / ID-passport with excerpts from a single state demographic register about registering the place of residence.
3. Certificate of recruitment to a call area / military ID (with a marking on the shelf life for military service and registering).
4. Certificates from a narcologist and psychiatrist (over the past 5 years).
5. Certificate of the passage of prophylactic drug inspection (form 140/0).
6. Medical certificate of passing a mandatory preliminary psychiatric examination (form 122-2 / 0).
7. Documented (with the seal of the medical institution and the surname of the Contractor) The results of the survey on syphilis (RW), hepatitis B, C, and HIV infection, general blood and urine tests, blood test for sugar, x-ray (fluorographic) study of the chest organs, Electrocardiography carried out within one month before a medical examination.
8. A detailed extract from the medical card of an outpatient patient over the past 5 years (place of residence, work, study, service), certified by the seal of the medical institution and the seals of doctors, and an outpatient map.
9. Information on mandatory preventive vaccinations in form No. 063 / O - 2 copies certified by the sealing institutions (issued in a medical institution at the place of residence, work, study, or service).
10. Exposures from medical documents of military units or medical characteristics (for former military personnel dismissed from service during the last year).
11. Extract from the order of dismissal from the service in the police, ZS.SBU, other military formations.
12. The resolution, certificate of illness (when dismissing from service or under health).

For the passage of the medical examination, candidates for training need to provide:

1. Directions to M (MM) C.
2. Photo 3 * 4 - 1pc.
3. Passport of a citizen of Ukraine / ID-passport with excerpts from a single state demographic register on the registration of the place of residence.
4. Certificate of assistant to a call area / military ID (with a marker about the shelf life for military service and registering).
5. Help from a narcologist and psychiatrist (over the past 5 years).
6. Certificate of the passage of prophylactic drug inspection (form 140/0).
7. Medical certificate about the passage of mandatory preliminary psychiatric examination (form 122-2 / 0).
8.
