SPA TREATMENT AND REHABILITATION

In accordance with the Order of the Ministry of Internal Affairs of 14.12.2015 № 1568 "On approval of the Order of preferential rehabilitation, sanatorium treatment, recreation and recreation in medical rehabilitation centers, sanatoriums, rest homes, boarding houses and recreational facilities of the Ministry of Internal Affairs of Ukraine policemen, other categories of persons and their family members "are entitled:

To free medical and medico-psychological rehabilitation:

• Police officers and employees of the Interior Ministry and the National Police of Ukraine (hereinafter - the National Police), who defended the independence, sovereignty and territorial integrity of Ukraine and directly participated in the anti-terrorist operation, ensuring its implementation and were directly in the areas of the anti-terrorist operation;
• Police officers and employees of the Ministry of Internal Affairs, injured in the performance of official duties and / or require continuation of treatment after in-patient treatment;

For preferential sanatorium treatment and recreation:

• police officers and members of their families (wife (husband), children under the age of 18, and in case of their education in higher educational institutions - under the age of 23);
• Citizens of Ukraine from among former police officers, who were dismissed from service due to health condition, age, in connection with staff reduction, who were assigned a pension in accordance with the Law of Ukraine "On Pension Provision of Persons dismissed from military service and some other persons", and their family members (wife (husband), children under 18 years, and in case of their study in higher educational institutions - up to 23 years) - once a year;
• Citizens of Ukraine from among former persons of rank and file of the internal affairs bodies, who were dismissed from service due to health condition, age, in connection with staff reduction, who were assigned a pension in accordance with the Law of Ukraine "On pensions for persons dismissed from military service, and some other persons" and their family members (wife (husband), children under 18 years, and in case of their study in higher educational institutions - under 23 years) - once a year
• family members of policemen who died (died) while serving in the police - once every two years on a gratuitous basis (wife (husband), if she (he) did not marry for the second time, children under 18 years old and in case of their study in higher educational institutions - up to 23 years old, as well as children - disabled from childhood (regardless of age), who were assigned a survivor pension in accordance with the Law of Ukraine "On Pensions for Discharged from Military Service, and some other persons")
• Family members of former servicemen and commanding officers of internal affairs bodies, who died in the line of duty - once every two years on a gratuitous basis (wife (husband), if she (he) was not married for the second time, children under 18 years, and in case of their study in higher educational institutions - under 23 years, as well as children - disabled from childhood (regardless of age), who were granted a pension on the loss of the breadwinner in accordance with the Law of Ukraine "On pension security of persons dismissed from military service and some other persons")
• civil servants and employees of educational institutions, medical, scientific institutions and support institutions of the MIA or the National Police from among former police officers, who as of the day of publication of the Law of Ukraine "On the National Police" served in the internal affairs bodies and had a calendar service of at least five years and continued work in the MIA or the National Police (their territorial bodies, the institutions and establishments) in the positions held by civil servants in accordance with the Law of Ukraine "On civil service", and in educational, medical, provision and research institutions - in any positions, and members of their families (wife (husband), children under the age of 18, and in case of their education in higher educational institutions - under the age of 23);
• Citizens of Ukraine from among former police officers, mentioned in the sixth paragraph of this paragraph, who have been awarded a pension in accordance with the Law of Ukraine "On pensions for persons dismissed from military service and some other persons", and their family members (wife (husband), children under the age of 18, and in case of their study in higher educational institutions - up to 23 years old)
• civil servants in accordance with the Law of Ukraine "On civil service" and other employees of bodies and institutions of the National Police and the Ministry of Internal Affairs, who do not belong to the categories of citizens specified in paragraphs one to eight of this item.

 In order to be registered for a sanatorium voucher, you MUST submit the following documents:

• Report / application for sanatorium treatment / rehabilitation and recreation;
• A copy of the service (pension) certificate and passport (for a wife (husband) - passport);
• Reference form № 070/O (issued by a doctor)
• A copy of a marriage certificate and a copy of a child's birth certificate - for family members;
• A copy of a certificate of a large family - for large families;
• Extract from the dismissal order;
• to pensioners - a certificate of the pension fund authorities about receiving a pension in accordance with the Law of Ukraine "About pension security of persons dismissed from military service and some other persons";
• To the victims of the Chernobyl accident a certificate from the Department of Labor and Social Protection of the Population about health resort treatment in the current year;
• certificate from the educational institution - for confirmation of the fact of full-time education in higher education institutions (for family members over 18 years old);
• copies of documents certifying the right to benefits.

When submitting copies of certificates, the originals are provided for verification.

It is obligatory to have your ticket with you when you receive it:

• The conclusion of the medical and selection committee;
• Medical certificate form 0.70 / 0;
• Sanatorium-resort card;
• Reference from the Main Department of the Pension Fund of Ukraine in Cherkasy region (Smelyanskaya St., 23, 5th floor, office 50) about being on the books in the main department of the Pension Fund of Ukraine, as a recipient of pensions for length of service in accordance with the Law of Ukraine "On pensions, retired from military service, and some other persons. (Decree of Cabinet of Ministers of Ukraine from 27.04.2011 № 446).
• Reference from the Directorate of Protection of the Population from the Consequences of the Chernobyl NPP Accident that you have not used the voucher in the current year (for participants of LPA on Chernobyl NPP of the 1-2 categories);
• Pension or service certificate;
• Document, confirming benefits;
• Passport (for family members);
• Employment record book (for working pensioners).
